This 56 year old Divisional Manager forest development corporation at Chamba HP,is an old case of Diabetes and high Blood pressure.On 9th Feb 2009 he developed severe chest pain and Angiography revealed severe Triple vessel disease.Blood tests also showed that due to Diabetes his kidneys had become very weak and his Urea was 160 (normal 20-40) and Creatinine was 3.5 (normal 0.6-1.2).He was advised immediate Bypass Surgery.But he was also advised by other doctors that with a damaged kidney his chances of death were very high and there was a 50% chance of further damage to his kidney needing dialysis for life.His EF was 35% which was another risk factor.With this information in mind he refused Bypass surgery and came to us for EECP+AMT.

We explained to him that there was a zero % chance of complications with our procedure and that his kidney would also benefit from EECP.We started treatment on 25 Feb 2009 and completed 35 one hour sessions .

His chest pain completely disappeared and he no longer had any shortness of breath.His urine flow returned to normal.What was really satisfying was that his kidney function improved beyond expectations.His Urea was 43 and Creatinene 1.3,both near normal.One month ago no doctor had given him even a remote chance of his kidney ever recovering normal function.His EF improved from 38% to 50% and he could now walk 2Km daily.

Dr Randhawa says We do not do EECP+AMT for renal failure alone.But patients of CAD who are unfit for CABG due to renal impairment have done very well and their kidney function has improved beyond all medical expectations.It is due to increased blood flow within the kidney tissues due to ECP.

    This 63 year old lady has Diabetes and High blood pressure since last 10 years.She has been having chest pain since 1998.In Feb 2004 she had an  Inferior wall Myocardial Infarction (heart attack) with a pulse rate of only 34 per minute and Blood pressure of 80/0 mm of Hg.A pace maker had to be put in at Tagore Hospital Jalandhar to save her life.By June 2004 her chest pain worsened and had breathlessness on minimal exertion, and she was then advised Bypass Surgery .Being a diabetic,physically very weak,having an EF (ejection fraction) of just 30%, she refused surgery.

    We started her on EECP+AMT on 10th November 2004.Her pacemaker gave us no trouble at all and the EECP machine triggering was perfectly normal.She completed her course of 35 one hour sittings.

    It has been Four and half year since we treated her.

    • She has never had chest pain since that day.
    • She has no difficulty in breathing while walking 1000 mtrs.
    • Pace maker is functioning normally.
    • Her EF today is 42 %

    Meet Dr YR Khanna 68yr old MBBS general practicioner and a retd Army Captain.He has to be a fore runner for the fighter of the decade award.Here is his fight to conquer CAD. He has the unfortunate distinction of having to undergone  angiography eight times.
    He first had angina in 1991.He went to Batra Hospital Delhi and an angiogram showed multiple blockages in his coronary arteries.They did ballooning at two places but did not put in stents as was the practice at that time.His relief was short lived and within a year angina had recurred in a big way.He got an angiogram repeated at All India Institute of Medical Sciences at New Delhi which showed that all the ballooned arteries had got blocked again.He then underwent a triple Bypass Surgery.The result was good and he remained free of pain for ten years.
    Recurrance of angina in 2003 made him consider all options including EECP.All physicians and cardiologists he consulted during the next two years dissuaded him from undergoing EECP.Some were completely ignorant;others downright critical about the procedure.
    The result was that in the next three years he had six stents placed in him in four of India's best hospitals with almost NO RELEIF AT ALL! And during this time he had spent a princely sum of Rs Fifteen lacs.
    In Feb 2008 after he had two DES stents placed in him without any relief at all, he revisited all the hospitals where he had had stents placed in him.They all said that there was NOTHING they could do for him now and he should remain on medical treatment for the rest of his life.But life was unbearable on medical treatment alone.He had severe angina after every meal.He had angina while walking to the bathroom.He had severe chest pain and choking in the throat even while changing his clothes.He had not attended his clinic since the last one week.Against all cardiologists advise he came to us for EECP treatment.
    Surprisingly he was on INADEQUATE medical treatment as evidenced by the numerous prescription chits he carried from all the top Hospitals in India. So the first thing we did was to start AMT or Aggressive Medical Treatment.This included full dose Statins,adequate beta blocade,addition of cardiac metabolic drugs,ACEIs,zero fat diet amongst others.
    On about the fourth day he had sufficient relief of angina to start attending his clinic.On the 10th day he walked up a flight of stairs without chest pain.On the 35th day he was walking 1000 meters twice a day with tremendous improvement in QOL(quality of life), and complete disappearance of accompanying depression.
    Today two months after completing his EECP course he says he never felt so good in the last 10 years.He has increased his walking to 1500 meters in the morning and evening.

    This lady had a history of HT-high blood pressure since 10years and DM-diabetes mellitus since 7years.Since last two years she started getting chest pain and breathlessness on exertion and later even at rest.

    She got her angiography done at Escorts Hospital Amritsar on 12th April 2008,which revealed all three arteries blocked at SIX different places from 70% to 90%.

    (RCA Proximal 90%, mid 80%. LAD prox 70%, mid 70%. LCX prox 90%, PDA prox 80%. Impression : Triple Vessel Disease, advised CABG).

    Her EF was 45%.She was advised immediate Bypass surgery.Now this patient had a very bad experience with two earlier surgeries (Hystrectomy and subsequent incisional hernia repair) due to her diabetes.She had repeated wound infection,delayed healing and prolonged recovery.In view of this she refused Bypass Surgery .

    On 2nd June 2008 she opted for ECP+AMT as it was completely safe for a diabetic and noninvasive.She went through the procedure smoothly and had complete relief of pain and breathlessness.Today she is back to her normal life.

    This 44yr old housewife is an old case of severe Hypertension(high BP)since 10years.In 2003 she started getting chest pain on exertion.This gradually worsened inspite of treatment.In Jan 2004 during peak winters in Amritsar when temperatures drop to Zero degrees,she got severe chest pain with sweating and a sense of impending death.She was admitted to a local hospital and diagnosed as Unstable Angina and severe hypertension BP 210/110.She was advised Coronary angiography which she refused.Chest pain got from bad to worse.She stopped socialising and had to discontinue her kitty parties.So much so she could not even walk to the kitchen due to severe chest pain.At the age of 40 she was practically bedridden. It was then that she decided to try out EECP which was new in Amritsar at that time.She completed the course of 35 one hour sittings along with Aggressive medical treatment.There was a dramatic transformation in her life.It was difficult for her to believe that the pain which was her constant companion for two years was no longer present.
    She is now an active socialite and leads a happy life with her businessman husband and two wonderful sons.She also actively advocates EECP to all heart patients. I must give back something to the procedure which has given me a new - LIFE WITHOUT PAIN.

    This 72 year old ex serviceman had severe chest pain on minimal exertion in September 2006.In Dec 2007 he suffered a major heart attack.On 31.Jan 2007  he got angiography done at Escorts Hospital Amritsar,which revealed blockages in the three arteries ranging from 60% to 100% in four different places.His heart functioning was affected and his EF was only 45%.He was advised immediate Bypass Surgery.His surgery would have been done free of cost-being an ex army man-even then he refused to undergo the risk in view of his age.
    Instead, he opted for the much safer EECP+AMT course at Randhawa Hospital Amritsar. We started his treatment on 10th Feb 2007 and he successfully completed 35 one hour sessions of ECP.His pain disappeared,chest felt very light and he started walking after an almost bedridden condition of 4 months.

    Conclusion:
    In this case a 72 year old with triple vessel disease needing IMMEDIATE surgery 18 months ago,is completely painfree and has not needed a second course of EECP till date.If this is not avoiding Bypass Surgery what is?

    This 39 year old housewife from Hoshiarpur suffered a major heart attack on 16th Jan 2007 (Extensive anterior wall MI).She was in a critical stage and ECHO showed a dilated heart(LV 64mm) with a very low EF 30%.She was stabilized and a DES Stent put in LAD on 22 Jan 2007 at Escorts Hospital Amritsar.She was pain free for about 6 months but continued to have breathlessness on walking within the house.Her condition gradually worsened and she was advised to undergo another Angiography with a view to do Bypass Surgery .Having had no relief with a stent,she refused the invasive option.

    She started with EECP+AMT on 12 March 2008 and duly completed 35 one hour sessions .Within 10 days her heaviness of chest disappeared and she could walk 100meters without breathlessness.

    With AMT- aggressive medical treatment she lost  4Kg weight  and her lipid profile improved as shown below.

    Surinder Singh a 63 yrs old had his first heart attack in 1991 and the second one in 1997. He was so incapacitated that he had to undergo CORONARY ARTERY BYEPASS GRAFTING(CABG)- Bypass surgery in 1998.

    After about five years he started getting chest pain on walking which rapidly worsened till mid 2005 when he used to get severe choking pain in the chest and throat on walking even 10 steps. His angiography was repeated on 08/08/2005 at Apollo Hosp Ludhiana which showed all arteries blocked and was advised immediate Bypass Surgery which HE REFUSED.He started EECP(ECP) at Randhawa Hospital from 26/08/2005 and completed 35 one hour sittings on  09/10/2005

    He can now walk one km without pain twice daily. His EF improved from 35% to 40% and his exercise time on TMT increased from5min to 7mins.
    With AMT advanced medical treatment his LDL decreased from 187 to 114mg% and TG from 157 to 92mg % in about 40 days. He has also lost 9kg weight in 4months. We conclude that patients who have had bypass surgery in the past can prevent a second surgery with EECP(ECP) and AMT.

    Diabetic since 6yrs with high BP since 1yr.Had severe chest pain for 7 days. Angiography done at BBC /Pruthi hospital Jalandhar on 09 Oct 06 showed all three arteries blocked 80%,80%70% and 70% at four different places. He also had serious ventricular arrhythmias.

    He was advised immediate Bypass Surgery .He did not want to take such a surgical risk so he chose EECP(ECP) at Randhawa Hospital, Amritsar starting from 12 Oct 2006.
    Today he is off all antidiabetic medicines and has never had chest pain since then. He has rejoined his duties as a BDO and walks three Km twice daily.

    On 16Sept 2005 ,14 days after starting EECP(ECP) his EF had improved to 28%.(An improvement of 65%; 28-17=11;11/17=65%).

    So EECP(ECP) is THE ONLY modality of treatment to improve EF- Ejection Fraction in a patient with failed Bypass Surgery who now has disabling Cardiac Failure. Patients who  are not yet disabled by Cardiac Failure (say EF of 30 to 50%) may take EECP(ECP) so as to pre empt and delay onset of Cardiac Failure. In all these patients EECP(ECP) definately improves the quality of life and avoids repeated Bypass Surgery.

    Balwant Singh a 65yr old farmer of village Buttar distt Moga, had severe chest pain on walking even 10meters. His weight was 88kg and BP was 180/110. He went to Hero DMC heart hospital where they did an coronary angiogram and told him that he had 100% blockage in two arteries and 90% blockage in the third. Effectively he had only 10% blood flowing in one of the three arteries supplying blood to the heart.

    The doctors told him that bypass surgery was the only way out or else he could die any moment. Like any one of us he was afraid of surgery and all its complications including death on the table. He started treatment on 7th Jan 2004 and took 35 sittings of ECP lasting 1 hr each.He was also given advanced medical treatment to open up his blocked arteries. He was started on special weight reducing diet.

    Suresh Khurrana a 49 yr old businessman from 135-Green Avenue, Amritsar, who suffered two major heart attacks in April 2004 and Oct 2004. He was advised bed rest at home since last two months. He had chest pain even while walking to the toilet and one night he had severe difficulty in breathing. He was shifted to the ICU and advised immediate Angiography followed by Bypass surgery. Since he did not want to undergo bypass surgery he did not see the point to undergo the pain and trauma of Angiography just" to see whats wrong".

    On 13 Oct 2004 his echocardiography report showed an ejection fraction of just 26% (normal 55-75%), indicating severe myocardial damage and dysfunction. Two different cardiologists opined that the only way out was bypass surgery. He chose EECP(ECP) and started treatment on 8th Nov 2004 and within 7days he was walking 200 meters without pain. After 15 days he had started walking one Km twice a day. At the end of 35 one hour sittings Mr Khurrana could walk 3 Kms twice a day. So Angiography is not a prerequisite for EECP(ECP). Those who chose the non invasive path can go for EECP(ECP) without any traumatic procedures like Angiography.

    Govardhan Das a businessman from Masit wali gali, Batala came to us with history of rapidly worsening angina and breathlessness since last 2yrs.He used to take 6 sorbitrate tablets daily, while going for urine, stool, bath and while changing clothes Angiography done at Tagore hospital on 13/07/05 showed all three arteries blocked with an EF of just 28%.Diagnosis Triple vessel disease with severe LV Dysfunction. He refused Bypass Surgery and we started him on EECP(ECP) and AMT on31/08/2005.

    After he completed 35 one hour sittings he did not have any chest pain or difficulty in breathing. He did not need sorbitrate tablets from after the seventh day of treatment onwards. His EF ejection fraction had improved to 55%. I think we have successfully avoided Bypass Surgery says Mr Govardhan Das.

    This 43yr old gram sevak suffered his first heart attack in Oct 2004. He got a second attack in March 2006.Angiography done at Escorts Amritsar showed two arteries blocked 100% and 80% with EF of 40%. It also showed an 80%blockage of Rt renal artery. He was advised Bypass surgery, but he refused because of the cost and fear factor.

    We started him on ECP on 26/08/06 and  he successfully conpleted 35hrs on 10/10/06. He was also given AMT or Aggressive medical treatment and graded, progressively increasing exercise programme.

    Today he feels he is completely disease free and healthy.

    One of the most respected intellectuals of Amritsar city had a massive heart attack in Jan 05.No Diabetes, No high BP.After the attack his EF fell to 20% only(Normal 55 to 75%).
